Our Outreach services are governed by a Service Level Agreement with Oxfordshire County Council to provide a medical needs teaching service.
Each September we will provide a detailed written report on the key measurements described in the SLA.
Referrals
Responding to referrals
Schools make referrals by completing a referral form and sending it to us via email or post.
We aim to acknowledge receipt of referrals within two working days.
Making a decision
Once a referral is complete (including the required information on attendance, SEN, medical evidence and headteacher’s signature) we can make a decision on the best way forward.
The referral may result in a rejection if the pupil does not meet our admissions criteria, or an offer to proceed on one of our pathways. These are:
- Assessment Phase Pathway – our Leader of Learning for Outreach will talk to all parties involved and write a detailed report recommending the way forward. This may involve one of our additional pathways or signpost other, more appropriate services.
- Pupil Facing Pathway– we will construct a plan that meets the needs of the young person and includes direct work with one or more of our teachers or higher level teaching assistants
- Professional Advice Pathway – we will advise the home school on ways to support pupils with the condition presented by the pupil they referred
We aim to make a decision within five working days of receiving a complete referral.
Starting on a pathway
Once a decision has been made on a pathway, preparations need to take place before work can begin. For example, for pupils on the ‘Pupil Facing Pathway’ a school SLA needs to be signed and returned to us, a home visit will take place and a detailed risk assessment is written. Liaison with mental health services may be required when designing a reintegration plan.
We aim to start the pathway within ten working days of a decision being made.
