Individual Healthcare Plans (IHP) – model template

Whilst there is no set template for an IHP The DfE’s guidance on supporting pupils with medical conditions, linked to above, lists examples of points for a governing body to consider when deciding what information to record in IHPs. These include:

The medical condition (and its triggers, signs, symptoms and treatments)

The pupil’s needs resulting from the condition (such as medication and other treatments, time, facilities, equipment, testing, access to food and drink, dietary requirements, and environmental issues)

Specific support for the pupil’s educational, social and emotional needs

The level of support needed

Who will provide the support

Who in the school needs to be aware of the child’s condition

What to do in an emergency

The government says that all plans should be created in partnership between schools, parents, and relevant healthcare professionals (for example, a paediatrician) and that pupils should also be involved whenever appropriate.  The plan should also name a lead professional from the school who is charge of monitoring and reviewing its success.
